Clinical Pharmacist

We have an exciting opportunity for an experienced Clinical Pharmacist to join our well-established forward thinking multi-disciplinary team.

You will have responsibility for overseeing all aspects of the practices Medicines Management services, including reconciling of patient medication on discharge from Secondary Care, acute queries, medication reviews, and auditing.  As well as providing expertise to the wider team for training and education.

Ideally you will you be an Independent Prescriber or working towards gaining the qualification and will act as a specialist resource to answer medicines-related queries, to signpost to other services and to liaise with other healthcare professionals as required.

You will have proven organisational skills and have the ability to communicate effectively across all teams, as well as with patients, families and carers. 

The role is full-time (37hrs per week), working on a Monday to Friday basis.
